Question on Ch 500 (Spoiler)

Questions regarding Tailed Beasts sealing/extraction and Naruto's history (Kushina?), because I could be forgetting something. Let's see if I'm right and if I can get a response before spoilers appear.

Here we go:
1.) When a tailed beast is taken out from a container, the container dies right?

2.) Is it possible that either:
- The Tailed Fox that Madara controlled to attack Konoha WAS a transformed or trapped KUSHINA, and she died when the 4th Hokage sealed the fox in (Dun dun, trauma battle). And that's why we see her soul during the last chapters?

OR

- or did Madara kill her by extracting the fox from her first? And controlled it then?

Anything mixed up?

I think it's the latter, and I think that we will learn about it in chapter 501.

judging by the fact that she is naruto's mom she probably will go into total berserker mode accidently and go ninetails. I think because madara is holding her barely 5 minute baby.
